If you are in the military and need a new ID card, be sure to bring the appropriate documentation with you to your appointment. The following is a list of what you will need:
-Security Card
-Voter’s registration card
-Original Birth Certificate
-Veterans Health Identification Card (VHIC)
-Citizenship or Naturalization Certificate
-State Issued ID (Drivers License, Non-Driver Identification Card, etc.)
The ID card office has set up an appointment system such that you must know your confirmation number to cancel or reschedule. If the information above does not pertain to your situation, please call the ID Card Office (904-542-1837) before arriving to ensure that you have all of the necessary documentation.
