Are you in the military and looking for a new ID card? Here’s what you need to know:

-You can make an appointment or walk in as a customer

-The MPF Customer Support Element is open from Monday to Friday, 8:30am to 3:30pm

-To cancel or reschedule an appointment, you will need your confirmation number

-Bring two forms of identification, one of which must be a state or government issued photo ID

-If your ID card was lost or stolen, you will need to provide a letter from your Command Sponsoring Agency

-To add a spouse to DEERS, you will need to provide the following documents: marriage certificate, birth certificate, social security card and a photo ID

-If the spouse is a foreign national, marriage certificate, valid passport and birth certificate in English translation are required

-For dependent ID cards, the sponsor must be present and sign the DD Form 1172

-If the sponsor is not available, the dependent can have a previously signed 1172 (within 90 days of signature) or power of attorney

-To add a child to DEERS, you will need to provide the following documents: birth certificate and social security card

-If the child is a stepchild, you will also need the marriage certificate

-For full time university and college students, you need a letter from your school registrar’s office stating they are enrolled full time in an accredited school in pursuit of an associate’s degree or higher; and two other forms of ID to have dependent benefits extended to age 23

-As for all military identification cards, appropriate civilian attire is required for dependents, retired, USNS Mariners, DoD civilian personnel and contractors
