The ID Card Division, DEERS and Rapids (Military Personnel Division) is located at Joint Base Lewis-McChord in Liggett Hall on Liggett Ave. Soldiers assigned/attached to the 7th ID, 593rd ESC, I CORPS, 1ST SFG and MAMC must report to their BDE S1 for CAC support. You will need two forms of identification, one of which must be an identity document (see the list on pages 9 of the I-9 form) in order to receive your ID card. If your ID card is lost or stolen, you will need to present documentation from the local security office or a memorandum from your CAC sponsor (someone in your command who has access to DEERS) confirming the ID card was reported lost/stolen. Required documentation for spouses and dependents is listed on page 2 of the I-9 form. If youre a full time student age 21-23, you will need a letter from the registrar of your school certifying you are enrolled in a full-time course of study leading to an associate degree or higher, and your anticipated graduation date. If youre adding a stepchild to DEERS, you will need a birth certificate for both the stepchild and parent, a Social Security card for the child, and the marriage certificate. If youre adding an illegitimate child to DEERS, you will need a birth certificate, social security card and a legal court order or paternity affidavit.
