If you have dependent(s) that are not registered in the DEERS system, here is a list of documents required to add/remove them from your record.
When adding a spouse to DEERS, the sponsor needs to bring the following documents for their spouse: -Marriage certificate -Valid passport -Social Security Card
When adding a child to DEERS, the sponsor needs to bring their child’s birth certificate and social security card. If their child is over 21 but not employed full time the sponsor also needs to have a letter from them stating they are enrolled full time in school.
If your spouse is a foreign national, you will need to provide their valid passport, visa and I-94 card. You will also need to provide an English translation of all documents.
If you are a civilian deploying overseas or are in an emergency essential status, you will need to bring two forms of ID as well as a completed DD Form 2365 and Letter of Authorization.
Please note, all documents must be translated into English and certified.
If you are a civilian who has recently moved please come in with two forms of ID as well as a certification from the U.S Postal Service that states your address has been updated. You will also need to bring documentation showing your new address.
If you have children that are not registered in the DEERS, but they are living with you and you would like them to be included, please bring a letter from your commanding office stating that they should be included.
If you have children and your spouse is not registered in DEERS either but would like their name added as well please bring a copy of their birth certificate or passport showing their name is included. You must also bring documentation showing your spouse’s current employment status such as a letter from their commanding officer or the most recent paystub.
If you have lost your DD Form 1172 please come in with two forms of identification and we will be able to issue you a new one. Please note that if it has been more than 90 days since your last appointment, you will have to go through the walk-in customer line.
