Are you a military retiree or dependent and in need of an updated ID card? Here are some important things to keep in mind when applying for a new card:
-You can now digitally sign the 1172-2 form using your CAC or DS log-in access.
-The “DEF” end date will now be issued with an expiration date. The front expiration date will now reflect the date of the end of Tricare/Begin date of Tricare for Life.
-If your CAC card has been stolen, you must bring a theft report from a Police Station. For lost CAC Cards, you must also bring a signed affidavit.
-If you are changing your name, you will need to do the following:
–Bring in an original marriage certificate or court order (if applicable) and 2 forms of ID from RAPIDS list. If you’re bringing in a non-current spouse’s CAC card for DEERS purposes only, you will also need to bring a copy of their birth certificate
-If you are a full time student, you will need to provide an enrollment letter from your school and sponsors must certify on the 1172-2 form that they provide over 50% support to the child.
-For foreign documents, all must have authentication certification. If a document is not in English, it will need to be translated and have an Apostille or Consular certification.
-If you are changing your name after marriage, you will need to provide original documents from RAPIDS list including a marriage certificate or court order (if applicable) and 2 forms of ID from RAPIDS list. If you’re bringing in a non-current spouse’s CAC card for DEERS purposes only, you will also need to bring a copy of their birth certificate.
