-You can now complete DD Form 1172-2 online.
-If your CAC card is lost or stolen, you must bring a theft report from a Police Station.
-When adding a spouse to DEERS, you must bring the following documents: Marriage Certificate, Birth Certificate, Social Security Card, and a photo ID.
-For first time DoD 100% DAV ID issuance, you will need DD214, VA Commissary/Exchange/MWR Award Letter, State or Federal Government Issued Photo ID and Social Security Card.
-If you are a full time college student, you must present a letter from your school’s Registrar stating that you are currently enrolled/registered as a full time student in pursuit of an Associate Degree or higher.
-For more information on the Tricare Young Adult program, contact TRICARE.
-To cancel or reschedule an appointment, you will need your confirmation number. Keep a copy of your appointment confirmation in case you need to cancel or reschedule.
-If the information above does not apply to your situation, please call the ID Card Office BEFORE arriving to ensure the DoD requirements will be met for your requested DEERS transaction to take place.
