Military personnel who are retiring and need to get a retiree ID card should know that there are several things they will need to bring with them to the ID Card Office. These include a copy of their DD Form 214, VA Award letter/Award Packet, State or Federal Government Issued photo ID and Social Security card. If they are a 100% Disabled American Veteran, they will also need to bring their DD Form 214, VA Award letter/Award Packet, and State or Federal Government Issued photo ID.
In addition, military personnel who have had their CAC card stolen must bring a theft report from a Police Station. For lost CAC Cards, documentation from the sponsoring agency reporting it lost is required.
When adding a spouse to DEERS, the sponsor must bring the following documents for their spouse: Marriage Certificate, birth certificate, social security card, and a photo ID.
Foreign documentation must have a certified English translation (if applicable) and an Apostille or Consular certification. Contact the document issuing facility to obtain authentication certification.
Military personnel must also know their confirmation number when cancelling/rescheduling an appointment at RAPIDS, which is located online at https://www.myidcardoffice.com/. Customers will need to know their confirmation number to cancel or reschedule an appointment.
